Review Lost Document

LBC Sum-ag told us today, July 19, 2021, that the ORIGINAL FORM 137 sent through them last June 17, 2021, is now DECLARED LOST. That document is an ORIGINAL RECORD of a child's schooling. You can't just lose it.
