Question for PET PLANS INCORPORATED

My father had paid his pet plans in 2005..we are living in koronadal and theirs no offices her..my father wants to claim his insurance..how will be able to claim his insurance?im his eldest daughter currently studying,,we want to avail it??can you please send faster the information because we need it.
