Question for HOLY TRINITY ACADEMY OF MALOLOS

I am an alumni of the high school department. I am looking to get my f137 as this is a requirement I need to submit to my current employer. How do I request for one if I am not in town? Can this be sent via email? Please advise. It would also be a great help if an email address is available.
