Question for B M I Slimmers World International

How can I refund my one time full payment of Slimmers lifetime membership paid and enrolled dated September 14 2005 under BMI ******** HO It was not been availed any services till now. I had a frozen shoulder and need to attend Physical Rehabilitation at Makati Medical Center. I had no work and retired from Corporate. Please help.
