Question for Nueva Ecija Doctors Hospital Inc.

I was born at Doctors Hospital on April 1, 1998 and an error exists in my birth certificate. My father's name was incorrect. We live in Pampanga and I'm asking this question for us to be prepared of whatever we need to get, a schedule or what not. What are the requirements for us to fix it? Please respond. Thank you!
